Our hours of operation are from 8:00 a.m. to 5:00 p.m. Eastern Time Monday through Friday. Please include the following information for verification purposes: Your first and last name Last four digits of your social security number Date of birth Profession License number *The system will automatically unlock your account within 24 hours.
